Because they describe an organisation the consultant observed for two days, and recommend changes nobody has time to make.
The gap between how work is described and how it is done is where most inefficiency lives. Reading a procedure tells you what someone intended; watching the work tells you what happens when the system is slow and a customer is waiting.
Change then has to be small enough to survive. One process, redesigned with the people who run it and documented in their language, beats a transformation programme that stalls at the second milestone.
And measurement should be modest. A handful of measures, defined once and reviewed monthly, produces more decisions than a dashboard with forty tiles that nobody opens.
